Pella Regional recognized as Level 3 Patient-Centered Medical Home

Pella — Pella Regional Health Center and their family of clinics in Pella, Bussey, Knoxville, Monroe and Sully was recently recognized as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance. This is the highest level of recognition possible, and only six facilities in Iowa have earned this recognition.

 

A medical home is designed to keep patients healthy by providing primary care, coordinating services needed and by taking steps to manage chronic diseases like heart disease and diabetes. The medical team partners with patients to assist in making informed decisions regarding their care and the care of their family.

 

“Primary care is the foundation of care provided at all Pella Regional Health Center medical clinics. Our primary care clinicians are frequently the first point of contact for a patient. These providers have a broad knowledge of health care conditions and many times follow their patients over many years, creating a high quality relationship that leads to better patient care,” said Lezlie Morris, chief of clinic services at Pella Regional Health Center.

 

Pella Regional Health Center has been working toward this recognition for the past two years. Recognition is granted by NCQA in three year intervals. Pella Regional will apply for recognition again in 2015.
